Using Your CHC Benefit Plan
When Visiting a Physician/Provider:
- Locate a participating PPO provider convenient to you by calling the PPO network shown on your member ID card, or visiting the “Carrier Directory” section of this website
- Schedule an appointment, and present your ID card when you arrive
- Your provider will call CHC to verify coverage and determine your out-of-pocket expense.
When Visiting a Hospital:
- Schedule and receive your required medical care/surgery (Pre-authorization is not required)
- The hospital sends all claims to CHC’s administrator for processing
- Applicable benefits are paid directly to the hospital
- In accordance with your plan benefits, any remaining charges are billed to you, and you are responsible for paying the hospital directly
When Visiting a Pharmacy:
- Present your ID card to a participating pharmacy to receive your pharmacy benefits
- If you have a “copay” plan, you will pay a set copay for your prescriptions in accordance with your plan
- If you have a “discount” plan, you will enjoy discounted rates on commonly used prescriptions
