Accessing your Master Policy Documents

Existing CHC clients now have access to their CHC limited fixed indemnity benefit plan documents on-line. Your specific plan information including, but not limited to, the following, is included with your Master Policy documents.

  • Employer Application
  • Benefit plan policies & certificates
  • Information on any Ancillary products specific to your CHC plan
  • Policies & procedures including adds, terms and claim processing guidelines
  • Sample ID card
  • Important phone numbers

To access your Master Policy documents, please click here. You will need to enter your “Email Address” and “Password” to enter this secure area of the website. (If your group is currently going through renewal, please contact your CHC Account Manager to confirm your most up-to-date Plan Documents are posted in this section of the website.) If you have any questions or are experiencing difficulty accessing your plan documents, please contact your CHC Account Manager for assistance.

Assisting your Employees with Questions

CHC Customer Service is here to help your employees with any questions they have regarding their CHC benefit plan. You may also direct your employees to refer to their member ID card for the name of the specific benefit plan carrier partner (PPO network, prescription drug plan, dental, vision providers, etc.), and refer them to the “Directory” link below where they can speak to a Customer Service representative for each line of benefits.

Filing a Claim

If your employees need to file a medical or ancillary (dental, vision, life, etc.) claim for reimbursement, please have them contact the CHC Customer Service department toll-free at 877.685.2432 or Contact Us via email.
