Existing CHC clients now have access to their CHC limited fixed indemnity benefit plan documents on-line. Your specific plan information including, but not limited to, the following, is included with your Master Policy documents.

(If your group is currently going through renewal, please contact your CHC Account Manager to confirm your most up-to-date Plan Documents are posted in this section of the website.)
